Children’s Care Management Services

Health Home Care Management and Early Intervention Services

 

Children’s Care Management is a FREE service available to qualifying Medicaid recipients 0-21 years old.

Care management is a community-based program. We meet you where you need us! We work with schools, collaborate with doctors and connect families with community services and resources. Our goal is for families to stay healthy and independent.

Health Home Care Management is a program in which all of an individual’s needs are addressed in a comprehensive manner. This is done through a contact person called a “care manager” who oversees and connects clients to all of the services an individual needs to ensure that they receive everything necessary to stay healthy, out of the emergency room and out of the hospital. A care manager will meet with the client and family at least once a month or as needed. Care managers assist with the communication among care providers, navigate systems, finding specialists, assisting with school needs and connecting families with appropriate community services, resources and transportation.

The Early Intervention program is established for individuals, birth the age three, who have or risk having a developmental disability. Children with certain diagnosed conditions at risk of developing serious delays, such as cerebral palsy and Down Syndrome, are automatically eligible for early intervention services. E.I. involves a close collaboration between parents, service coordinators, evaluators and the E.I. team. E.I. offer an array of evaluations and services with the purpose of maximizing the full potential of each child. It is about early identification and ensuring that individuals obtain the quality care that they need. Most services are provided in the child’s home or daycare setting.

Who can get these Services?

Children and Youth (under age 21) who are covered by Medicaid and have mental health and/or substance use needs, can get Children and Family Treatment and Support Services at no additional cost. These services are covered by regular Medicaid or, if enrolled in a medicaid Managed Care Plan*, these services are available thorough the health plan.
